Additional Participant registration form uses different mark-up for price sets

Compare Register.tpl to AdditionalParticipant.tpl. This makes it difficult to add javascript to show/hide price set options, for example.

See attached patch for a possible solution. Caveat: Not thoroughly tested. I have no idea if there is some reason that this tpl can't use in the PriceSet.tpl include.
