CRM-6844 Allow templates to be customized based on profile group name

This avoids the use of ID's and hence makes module code a lot more portable across versions and across installtions.

At a later stage we might also want to use the same mechanism for customizing event pages and contribution pages. We'll need to introduce the "name" field for those pages.
