DirectDebit fields missing in Contribution, Membership, Renew membership

      Description

      When using a DirectDebit payment processor, the corresponding fields do not show up. Here I provide a patch which solves this problem.

      I would be pleased if the patch could be integrated into the trunk as soon as possible - just let me know if there is anything else I can do for it.
