Constituent Summary issues with new order by features

1. Constituent Summary implementation for Order By - we are setting invalid default values for this report
1.1 order_bys_1_order is currrently DESCENDING for the default Last Name, First Name column. Should be ASCENDING by default
1.2 Create section header (order_by_section_1) should NOT be checked for Last Name, First Name column by default (this creates a section header for each contact in the report which is superfluous and confusing).

2. If another Order by Column is added and the Preview button is clicked, the additional optionField$i rows are not displayed when the form is reloaded. Same bug applies if report is saved with > 1 column header. When first loading the report, all populated rows are shown - but if you click Preview and open up the Criteria pane again, only row 1 is shown.

3. Currently the report allows these Order By columns: sort_name, email, country plus all searchable custom fields. We need to offer these additional Order By Column options to make this useful for NYSS:

• State/Province
• City
• Postal Code

4. Rename Order by Column - 3rd column table header.
Current header title: "Create section header"
New header title: "Section Header / Group By"
