Change the profile structure

1) add a title column

Now the name of the field (interal for admin users) is the same as the title (external, used on the end-user forms)
It leads to a lot of unclear names ("your contacts" "more about you" "specific requirements") that are not very useful when you are trying to choose a profile

Don't care about the naming of these fields (name vs. description?), the idea being that one is used when displayed and the other is private to manage the profiles.

2) display the places where the profile is used(dgg good analogy with the priceset) as stored in uf_join table
