Event Cart checkout shows unlabeled checkbox for Pay Later although option is disabled

While doing a quick test for CRM-14268 with sample data in a current git checkout from master branch, noticed this bug.

The "is_pay_later" checkbox is showing on the Cart Checkout form above the Credit Card Info block WITHOUT a label. I had Pay Later unchecked in the event's Fees configuration tab so this checkbox should NOT be present on the checkout form. (screenshot attached).

Also, I'm not sure whether the Conference Slot feature is working in 4.5. I thought I added some slots in the Conference Slot tab, but I don't see any changes or outputs in the DB, in the configuration tab, nor when I use online registration.

Finally, the default profile for events as of 4.4 includes email address, so the hard-coded email address in the cart registration form should be removed.
