Adjustments to UK civicrm_state_province's counties

Some UK counties that are out of date:
Clwyd: no longer exists

These, however, do exist:

• Anglesey: http://en.wikipedia.org/wiki/Anglesey
• Conwy: http://en.wikipedia.org/wiki/Conwy_County_Borough
• Denbighshire: http://en.wikipedia.org/wiki/Denbighshire
• Flintshire: http://en.wikipedia.org/wiki/Flintshire
• Wrexham: http://en.wikipedia.org/wiki/Wrexham

There is a list of counties here that appears to be up to date: http://digimap.edina.ac.uk/webhelp/os/data_information/os_products/supporting/county_abbreviations.htm
