Localisation screen "Available States and Provinces" list

2 Issues and one mild grumble here:

1. The "Available States and Provinces" list on the localisation configuration screen shows a list of countries in the states field.

2. (only for 4.7+) The method of selecting countries and states here makes it very slow to select lots of states and countries easily.

3. Countries are now all UPPPER CASE which is a bit ugly on the user interface but it seems it was done this way for mailing reasons... https://issues.civicrm.org/jira/browse/CRM-16876
