More warnings for case activities with mismatched date/status combinations

As a follow-up to CRM-4379:

(a) It would be useful if the warning appeared for newly entered case activities too, not just when editing existing ones.

(b) A similar situation would be even more useful to have the warning also, specifically when you create a new case activity, mark the status as completed, but forget to fill out the actual date.
