            Description

            Building on the work of CRM-11468, the new form-designer UI should be embedded within the "New/Edit Survey" screen.

            Modify tab layout shown in mockup to combine the 2 profile tabs into one tab: "Contact Info and Questions". The form in that tab will contain both 'sets' of profile management widgets:

            Contact Info [ - select profile - ] Preview Edit Copy Create New

            Survey Questions [ - select profile - ] Preview Edit Copy Create New
